Confidentiality – I agree that is it my responsibility to ensure that the user name and password assigned to me for the purpose of accessing the Eye Health Manager portal located on the Envolve Vision (Envolve Vision ) website will be maintained in confidence and only used by me and/or my employed staff. I agree that it is my responsibility to change my password immediately if any staff member who has accessed the Eye Health Manager portal terminates employment with me for any reason. In the event my password is compromised in any way, is used by or disclosed to an unauthorized person, or is used in an unauthorized manner, I will immediately change my password and report the incident to Envolve Vision at 800-531-2818. I agree that I am solely responsible for the use of my user name and password and of any disclosures resulting therefrom, and that Envolve Vision is specifically not responsible for the use of my user name and password by me or another party, except and only to the extent due to the negligence of Envolve Vision . I understand that Envolve Vision may change my user name and password at any time without giving me prior notice.

Unauthorized Use – I acknowledge that any unauthorized use of my user name and password, or of any unauthorized use of information obtained through the Eye Health Manager portal as the result of the use of my user name and password, may be grounds for immediate termination of my Provider Participation Agreement.

Termination – Should my Provider Participation Agreement terminate with Envolve Vision , I acknowledge that these terms and conditions, including my right to access the web site, will automatically terminate the date my Provider Participation Agreement termination becomes effective. I further acknowledge that Envolve Vision reserves the right to terminate these terms and conditions, including my right to access the web site at any time without notice at Envolve Vision 's sole discretion.
